I am a member of Enterprise Credit Union and am eligible to join the Member Protection Plan.

■ I have read and understood the Terms of Cover as provided and available to me.
■ I understand that the Member Protection Plan (MPP) is provided under a group life insurance policy, where Enterprise Credit Union is the policyholder and I am an insured member under the plan.
■ I understand that the fixed benefit payable under the Programme for the current policy year (1st April 2026 to 31st March 2027) is £5,000.
■ I understand that in the event of a valid claim, the benefit is paid to the Credit Union and deposited into the deceased Member’s Share Account.
■ I understand that the annual cost of the Programme is £80 and I will undertake to have sufficient available funds in my credit union shares account to pay for the cost attaching to my inclusion in the Programme.
■ I understand that this is a Programme that will auto renew on an annual basis, and the respective annual cost will be deducted from my credit union shares account at the start of each Policy Year or on a pro-rata basis where applicable.
■ I am also aware that the benefits and costs of this Programme may be varied annually. I acknowledge that I will be informed of any such changes or updates prior to renewal. This communication will be made through the website www.enterprisecreditunion.org
and all other communication channels of Enterprise Credit Union.
■ I’m aware that this is an annual programme, which runs from 1st April 2026 to 31st March 2027 and renews annually thereafter. I acknowledge that the MPP Programme’s ongoing renewal is subject to the terms as set-out in my Terms of Cover document.
■ I understand that either the insurer or Enterprise Credit Union can opt to terminate the Programme and cover at the end of each Policy Year.
■ Should I wish to opt–out or cancel my participation in the Member Protection Plan, I will inform Enterprise Credit Union in writing by giving at least one week’s notice of my decision. I will receive a refund for the unused portion of my participation, calculated on a daily basis, for the period for which I am no longer a member of the MPP.
■ My understanding is that my participation in the Programme will automatically stop at the end of the calendar month in which I cease to be a member of Enterprise Credit Union or where there are insufficient available funds in my account to cover the cost of my premium.
